About The Position

Mercy Community Healthcare has an opening for a Mental Health and Social Services Case Manager in Franklin, TN. We are looking for enthusiastic people with a passion for making a difference in the lives of those we serve in our community. This position provides case management services to patients and their families, addressing medical needs as well as social determinants of health. We provide care to the uninsured, underinsured and insured through pediatric and adult primary care, mental and behavioral health services, and care coordination.

Requirements

  • Bachelor’s degree in social work field preferred, but a degree in a relevant field is required.
  • Must become a certified application counselor once employed and trained.
  • One year of case management experience or work in healthcare (EMR) is required.
  • Demonstrated ability to multitask and handle prioritization with productivity.

Nice To Haves

  • Background in team-based environment or leadership role preferred.
  • Spanish-speaking preferred.

Responsibilities

  • Provides individualized, needs-driven case management services to patients and their families, including connecting the patient and family to internal and external services that improve overall physical, mental and spiritual well-being.
  • Meet face-to-face with clients and families to complete a comprehensive needs assessment addressing physical and socioeconomic drivers of health.
  • Assist with paperwork and online applications, facilitate scheduling with external offices for referrals, and advocate for the patient with the care team.
  • Perform telephonic case management as well, calling clients and offices to meet needs as they arise.
  • Document all relevant information within the EMR.
  • Research psych and behavioral health referral sources to keep updated resource log; help to develop relationships with partner agencies.
  • Serve as connection point for clients and the community in accessing BH and social services including residential, IOP/PHP, SUDs, SNAP, transportation aid, and financial assistance programs.
  • Represent Mercy at community events and serve as a direct contact for connecting clients to wraparound services.
  • Help patients apply for health insurance benefits and financial programs, as applicable, and screen them for available assistance programs to decrease barriers to care.
