About The Position

Ravenwood Specialty Care, a 176-bed long-term care skilled nursing community in Waterloo, Iowa, is hiring a Memory Program Coordinator (CCDI). The role plans, implements, and evaluates memory care programming for residents with dementia and cognitive impairment, using person-centered approaches to support engagement, independence, safety, and quality of life. The coordinator collaborates with care teams, residents, and families and helps ensure programming and documentation meet applicable regulatory standards.

Requirements

Experience working with residents or patients with memory or cognitive impairment.
Working knowledge of federal and state regulations governing long-term care.
Commitment to treating others with empathy, respect, patience, and discretion.

Nice To Haves

Bachelor’s degree with a concentration in Therapeutic Recreation, Nursing, or Social Work.
Certified Therapeutic Recreation Specialist (CTRS) credential.

Responsibilities

Plan, implement, and evaluate memory care programming for residents with dementia and cognitive impairment.
Provide person-centered interventions that promote engagement, independence, and quality of life.
Partner with nursing, social services, therapy, and activities teams to support individualized resident care plans.
Monitor resident responses to programming and adjust approaches to changing cognitive and behavioral needs.
Educate and support staff on dementia care best practices and communication techniques.
Support families through education, resources, and participation in care planning and memory program activities.
Ensure programs and documentation meet federal, state, and facility regulatory standards.
Document and report participation, outcomes, and program effectiveness.
Promote a safe, structured, and therapeutic environment for residents with memory loss.
