About The Position

The Memory Care Program specializes in diagnosing, treating, and caring for neurodegenerative conditions affecting the aging nervous system. The Memory Care RN provides expert care to patients with memory loss, dementia, cognitive dysfunction, and Alzheimer’s disease. Key responsibilities include coordinating care, medication management including refill requests, assisting with prior authorizations, telephone triage, and patient/family education. You will collaborate with the treatment team and ambulatory techs to prepare patient visits and review referrals for new patients to determine next steps. This role offers a unique opportunity to make a meaningful impact on patients and their families in a highly specialized and rewarding field.

Requirements

RN license
Experience in memory care or related field (implied by specialization)
Ability to coordinate care
Ability to manage medications
Ability to assist with prior authorizations
Ability to perform telephone triage

Responsibilities

Provision of competent direct patient care based on established standards.
Contributes to unit activities and supports enhancement of the care delivery system at the unit level.
Supports peers in the delivery of patient care services and assumes an expanded role which may include charge nurse, preceptor, and committee member responsibilities.
