About The Position
The Memory Care Coordinator works with the interdisciplinary healthcare team to provide comprehensive care and support for residents with dementia and dementia-related conditions. The coordinator plans, organizes, implements, and oversees specialized dementia programming aligned with best practices in cognitive wellness. The role supports residents’ emotional, cognitive, physical, and social well-being through structured, individualized, and compassionate programming.
Requirements
- At least two years of experience in long-term care or memory care programming as a Certified Occupational Therapy Assistant (COTA) or in another discipline specified in the posting.
- A Dementia Practitioner certification, or ability to obtain it within six months of hire.
Nice To Haves
- Certified Occupational Therapy Assistant (COTA) qualification.
- Associate or bachelor’s degree in Human Services, Nursing, Occupational Therapy, Recreational Therapy, or a related field.
- Current certification as a Dementia Practitioner.
