About The Position

Westside Care Center is seeking a social worker to serve as its Memory Care Coordinator. The role coordinates, supervises, and facilitates the delivery of social work services to facility residents, whether provided by facility staff or contracted staff, in accordance with applicable state and federal regulations and professional standards. The position focuses on programming and services for the Memory Care Program.

Requirements

Bachelor’s degree in social work or a related human services field.
At least one year of experience in long-term care providing direct social services is preferred but not required.

Responsibilities

Conduct face-to-face resident interviews and assess cognitive, mood, behavioral, and other psychosocial factors.
Participate in planning resident discharges.
Respond to clinical emergencies, regulatory agency visits, surveys, and issues outside normal work hours as needed.
Direct and supervise medically related social services for residents and, as needed, their family members and significant others.
Participate as needed in the clinical evaluation and screening of potential facility admissions.
Collaborate with the facility interdisciplinary treatment team to access information about community-based services and make referrals as appropriate for residents who wish to access those services.
Serve as a facility treatment team liaison with contracted mental health practitioners and consultants.
Participate in developing and implementing behavioral and psychosocial service programs and presenting those programs to physicians, family members, and residents.
