Requirements

  • Graduate of an accredited school of pharmacy.
  • A minimum of five years clinical practice experience in a hospital or health system pharmacy practice (or four years plus a year of residency training) required.
  • Keyboard skills and ability to navigate electronic systems applicable to job functions.
  • Current Registered Pharmacist License (RPh) for applicable state required.
  • ACLS and/or PALS required within three months of hire.
  • Completion of ASHP or ISMP Medication Safety Certificate program within 3 years of hire.

Nice To Haves

  • Advanced degree (MS or PharmD) and/or residency preferred.

Responsibilities

  • Coordination of the activities involving medication safety in the organization.
  • Provides leadership for medication safety assessments.
  • Educates providers and staff on the system-based causes for medication errors.
  • Communicates best practices regarding effective medication safety strategies to others within the system.
  • Interfaces with hospital site and system pharmacy and quality representatives on medication safety issues.
  • Review, analysis and trending of all medication events for the system.
  • Participation in any root cause analysis involving a medication event.
  • Creation or review/revision of patient care policies that impact the safe use of medications.
  • Elevate the importance of medication safety in the Legacy Health.
