About The Position

This position works independently and as part of a team to reconcile patients’ medication regimens during the admission process. Medication reconciliation involves obtaining a complete and accurate medication history with the patient and/or family, comparing home medications with medications ordered on admission, identifying discrepancies, and clarifying any discrepancies noted.

Requirements

• High school diploma or equivalent.
• Pharmacy Technician registration through the Alabama Board of Pharmacy.
• National Technician Certification recognized by ALBOP.

Nice To Haves

• College courses are desirable but not required.
• Two years of experience in a clinical setting preferred.
• Pharmacy experience preferred.

Responsibilities

• Obtain complete and accurate medication histories with the involvement of patients and/or their families.
• Compare patients’ home medications with medications ordered on admission.
• Identify medication discrepancies and clarify discrepancies noted.
