The Medication History Specialist (MHS) is an integral member of the Emergency Department team and works with patients identified for hospital admission to obtain and document the best possible medication history. The role helps promote safe, high-quality care by reducing medication duplications, omissions, and potential errors. Medication histories are entered into the electronic medical record and used by physicians, pharmacists, nurses, and other healthcare providers to inform treatment plans, medication orders, and discharge planning. The MHS gathers and clarifies medication information through patient and family interviews, prescription-bottle review, and contact with community pharmacies and physician offices. The specialist communicates relevant history to healthcare professionals, monitors the Emergency Department tracking board for potential admissions and complex patients, and updates the home medication record after admission to reflect ordered or active prescription medications. The role works under the direction of Pharmacy and Emergency Department leadership, the Medication Safety Officer, and nurses as needed to resolve discrepancies. It also requires compassionate, professional service to patients and families, along with successful completion of medication-reconciliation, annual competency, and quality-assurance requirements.
