About The Position

The Medication Historian is responsible for obtaining an accurate and complete admission medication list through face-to-face interviews with patients. This includes prescription, over-the-counter, and herbal products, noting drug name, dose, frequency, compliance, and last dose taken. The role also involves documenting any known drug allergies and reactions, which may require collaboration with family, physician offices, nursing homes, or retail pharmacies. The position requires effective communication, advanced interviewing skills, problem-solving, critical thinking, and advanced computer skills for information gathering and documentation. Adherence to all laws and regulations pertaining to patient health, safety, and medical information is essential.

Requirements

  • A high school diploma or equivalent is required.
  • Must be a registered pharmacy technician or be eligible for technician/trainee registration in the State of Iowa. This must be completed prior to start of employment.
  • National Pharmacy Technician Certification required within 6 months of employment.
  • Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.
  • Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.

Nice To Haves

  • Two or more years of experience as a pharmacy technician (retail preferred) or allied health professional, with knowledge of medications including brand/generic names, available dosages, frequencies and indication.

Responsibilities

  • Obtains an accurate and complete admission medication list via a face-to-face interview with patients to include prescription, over the counter, and herbal products noting drug name, dose, frequency, compliance, and last dose taken.
  • Obtains and documents any known drug allergies and reactions.
  • Collaborates with alternative information sources such as family, physician office, nursing home, or retail pharmacy.
  • Communicates effectively and uses advanced interviewing skills to conduct the medication history interview with patients in a manner that is professional and non-invasive.
  • Problem solves and applies critical thinking skills in obtaining admission medications lists for a wide variety of patient populations.
  • Applies advanced computer skills to gather information and document the patient’s admission medication list.
  • Knows and adheres to all laws and regulations pertaining to patient health, safety, and medical information.
