About The Position

Saint Joseph Health System is seeking a part-time Medication Historian or Pharmacy Technician to assist with the accuracy and completeness of medication history collection across the continuum of care. This role begins in the emergency department or inpatient units and operates under the direct supervision of the pharmacy department. The Medication Historian will work with patients, families, or caregivers to establish the most accurate medication history and provide support to physician and pharmacy staff when clarification of home medications is needed. This position also involves comparing electronic medication records to patient-provided information to ensure accuracy.

Requirements

• HS Diploma or GED equivalent
• Licensed by the Indiana State Board of Pharmacy Tech as a Pharmacy Technician.
• Four (4) years' experience as a pharmacy tech required
• Excellent written and verbal communication skills with people from diverse backgrounds and at all levels within the medical center and in the community.
• Ability to create and foster a cooperative teamwork environment.
• Basic computer skills, electronic data entry.
• Knowledge of commonly used prescription drugs including brand/generic names, available dosage, frequencies and indications.

Nice To Haves

• Ambulatory or retail experience preferred.

Responsibilities
