Redwood Coast PACE with Humboldt Senior Resource Center is looking for a full-time Medication and Discharge Coordinator. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults. At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health. The schedule for this position is Monday through Friday from 8:30 am to 5:00 pm. JOB SUMMARY: Under administrative direction and supervision of the Clinical Nurse Coordinator, responsible for aiding the clinic team with medication coordination and discharge planning of Redwood Coast PACE participants. Responsible for maintaining pharmacy database as well as ordering, distribution, and discontinuing of participants medications per provider orders. Maintains all required documentation for medication and treatment administration. Collaborates with the interdisciplinary team to develop and implement effective discharge plans for PACE participants who are being discharged from various healthcare settings, such as hospitals, rehabilitation centers, or skilled nursing facilities. Position works closely with participants, their families, caregivers, and community resources to facilitate a safe and successful transition back to their homes, and to promote optimal medication adherence.
