About The Position

Under direction, oversees the resident discharge process for the Sub-Acute Unit and performs the work involved in providing needed social assistance.

• Participates and takes the lead in the utilization review process for the Sub-Acute Unit
• Prepares residents for discharge
• Performs insurance reviews
• Issues cut letters for Medicare clients
• Participates with staff relevant to diagnosis and the design of effective treatment plans
• Provides social services to assist residents in the adjustment to their environment
• Reviews cases of admissions
• Interviews patients to assure that certifications are in accordance with hospital criteria
• Reviews records, data and information to assure that re-certifications are in accordance with needed extended care
• Analyzes and evaluates data bearing on social and/or emotional problems residents are experiencing in properly adjusting to various situations
• Utilizes family and community resources
