Mosaic Life Care provides Inpatient Rehabilitation/Post-Acute Care in St. Joseph, MO. Therapy plays a very important role in recovery from a life-altering injury or illness. We offer a hospital-based physical rehabilitation program for the treatment of those who have experienced a debilitating injury or illness. Our Unit is a very fast-paced yet fun environment with endless opportunities to learn and grow! We are looking for a dynamic Discharge Planner who is passionate about helping others and is a team player!

Requirements

  • Handle With Care within 30 days of hire.
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Nice To Haves

  • Bachelor's degree is preferred or 3 years of experience; masters is preferred.

Responsibilities

  • Communicates effectively and routinely with patients, families, medical staff, nursing staff, other caregivers when arranging follow-up care.
  • Keeps track of all Medicare forms and consent forms.
  • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Assisting with group activities.
