About The Position

Under direction, oversees the resident discharge process for the Sub-Acute Unit and performs the work involved in providing needed social assistance. This role participates and takes the lead in the utilization review process for the Sub-Acute Unit, prepares residents for discharge, and performs insurance reviews. The Coordinator also provides social services to assist residents in adjusting to their environment and reviews cases to ensure certifications and re-certifications align with hospital criteria and extended care needs. Additionally, the role involves analyzing social and emotional challenges residents face and preparing them for discharge by utilizing family and community resources. Other duties include attending training and preparing records and reports.

Requirements

  • Possession of a Masters Degree in Social Work from an accredited facility.
  • Possession of an LPN or RN license from the New Jersey Department of Health and Human Resources.
  • Four years of professional social casework experience or four years of experience in clinical nursing which shall have included experience in the admission and discharge process.

Responsibilities

  • Oversees the resident discharge process for the Sub-Acute Unit.
  • Participates and takes the lead in the utilization review process for the Sub-Acute Unit.
  • Prepares residents for discharge.
  • Performs insurance reviews and issues cut letters for Medicare clients.
  • Participates with staff regarding diagnosis and treatment plan design.
  • Provides social services to assist residents in adjusting to their environment.
  • Reviews cases of admissions and interviews patients to assure certifications are in accordance with hospital criteria.
  • Reviews records, data, and information to assure re-certifications are in accordance with needed extended care.
  • Analyzes and evaluates data bearing on social and/or emotional problems residents are experiencing in properly adjusting to various situations.
  • Utilizes family and community resources for discharge planning.
  • Attends required in-service and other training and development.
  • Prepares records and reports.
