About The Position

S:US is seeking a Medication Coordinator for our mens shelter located at Ralph Avenue, Brooklyn, NY 11233. This position is responsible for administering medications and documenting the administration as required, monitoring MARs and medication storage, and working with residents and Case Managers in filling prescriptions and ensuring refills are ordered as necessary. The Medication Coordinator will also ensure physician orders are current, properly documented and communicated to staff, monitor client medical equipment for safety and proper functioning, and attend and participate in regular staff training and scheduling meetings. Maintaining confidentiality and privacy regarding all clients’ information, completing daily documentation of all agency required material, and completing incident reports in accordance with policy and procedures are also key responsibilities. The role requires understanding and implementing the agency’s philosophy, policies and mission, and following the agency chain of communication. Ensuring the safety and well-being of the clients on a daily basis, working cooperatively with other team members to carry out program goals and objectives, and reporting any actual or suspected abuse/neglect in accordance with policy and procedure are essential. The Medication Coordinator will assist the psychiatrist and program nurses, prepare clients for self-medication status and monitor as needed, and make health assessment referrals. Compliance with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes is mandatory. Maintaining CPR/First Aid Certification, assisting in building evacuation during fire drills and building emergencies, and referring residents to emergency care if needed, working with emergency responders/emergency room staff to coordinate emergency treatment, and coordinating treatment and discharge planning with hospital staff are also part of the role. Additional duties may be assigned.

Requirements

  • High School Diploma or GED
  • At least one year of experience in direct clinical work with mentally ill populations.
  • Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
  • Must have ability to supervise staff and promote team building; good problem-solving skills, good written and oral communication skills
  • Strong computer skills in Microsoft Word, Outlook, Excel and PowerPoint
