The Medication Coordinator (LPN) is responsible for coordinating medication-related services and supporting nursing operations within Minnesota Adult & Teen Challenge ("MnTC") programs. Working under the direction of a Registered Nurse (RN), Supervisor, Nursing Services, or other designated clinical leader, this position serves as a medication-focused nursing resource, assisting with medication management processes, medication reconciliation, client education, documentation, provider communication, and continuity of care activities. The Medication Coordinator (LPN) serves as a resource for medication-related matters and helps ensure compliance with applicable nursing standards, Minnesota Department of Human Services (DHS) requirements, Minnesota Department of Health (MDH) regulations, and MnTC policies. This position supports individuals receiving substance use disorder and behavioral health services while promoting client safety, wellness, and treatment outcomes.
