We are seeking someone who is compassionate and detail-oriented to join our team and support healthcare professionals in providing top-notch medical services to our community. Responsible for care coordination of Medication Assisted Recovery patients. Facilitates communication between MedLink providers, accountability court counselors, recovery care organizations, and MAR patients.

Requirements

  • Ability to establish positive relationships with co-workers, professional staff and management.
  • Sound interpersonal skills and the ability to interface with all levels of staff and management.
  • Ability to establish and maintain relationships with employees, elected and other government officials, members of neighborhood organizations, social and medical professionals and the general public.
  • Prior experience working with patients with substance use disorder or mental health diagnosis.

Nice To Haves

  • Completion of Medication-Assisted Treatment Specialist certification or other similar certification preferred.

Responsibilities

  • Review patients in the clinic daily, participating in patient assessment and preparation, including day-to-day program concerns, and ensuring education and counseling has been completed by providers
  • Enroll patients into the treatment program and schedule initial assessment
  • Support the patient and provider in the program initiation, stabilization, and maintenance treatment procedures
  • Assist members of the health care team in the plan of care
  • Assist the program provider in making referrals to community providers for counseling or higher levels of care when needed
  • Maintain therapeutic relationships with the patient and the provider
  • Communicate with providers, drug court counselors, clinical staff, and referral sources to coordinate care for MAR patients through the various levels of care.
  • Care coordination including medication and treatment monitoring
  • Insurance authorization and troubleshooting
  • Relationship building and patient linkage to additional support (drug treatment services and mental health care)
  • Relationship building and facilitation of ancillary services
  • Share information with other members of the health care team and evaluation team through chart documentation, interdisciplinary team meetings, and email
  • Coordinate MAR medication issues, including consent, authorizations, and administration of medication.
  • Communicate with MAR Program Coordinator regarding such issues.
  • Work with the Provider, Practice Manager, Practice Site Supervisors, Front Office Assistants, and Clinical Staff to prepare for patients presenting to the office for MAR initiation and follow-up.
  • If lab/diagnostic imaging or specialist referral is noted on the visit summary, obtains results if not in EMR. Obtains reports if reason for visit is noted as hospital or emergency department follow-up.
  • Obtain informed consent from patients prescribed by the provider
  • Review the medical record to determine care plan ordered by the provider
  • Provide patient counseling regarding medications, barriers to care, drug screening, follow-up appointments
  • Identify and link patients with community resources to facilitate referrals and respond to social service needs.
  • Complete screenings for the provider as needed
  • Review treatment goals with patients.
  • Provide relevant self-management support for enrolled patients
  • Assist patients with the process of determining eligibility, obtaining and maintaining pharmaceuticals
  • Establish and maintain relationships with identified service providers and community resources
  • Demonstrate excellence in both internal and external customer service
  • Understand and effectively communicate HIPAA compliance, corporate compliance and client confidentiality
  • Ensure compliance with local, state, and federal regulations
  • Serve as a resource for center staff for MAR protocols and guidelines
  • All other duties as assigned
