About The Position
VitalCore Health Strategies (VCHS), an industry leader in Correctional Health Care, has openings for an MAT Coordinator Licensed Practical Nurse (LPN) at Sussex Community Correctional Center in Georgetown, DE. This role is part of a team with the MAT Case Manager to manage all aspects of patient care for incarcerated individuals receiving Medication Assisted Treatment for Opioid Use Disorder and deliver a recovery-oriented system of care. VCHS promotes a positive work environment based on respect and appreciation for staff dedication.
Requirements
- Must have and maintain current licensure as a Licensed Practical Nurse in Delaware.
Responsibilities
- Medication Inventory Management
- Initial and ongoing SUD (substance use disorder) assessment and evaluation
- Patient Care Coordination
- Collaborate with community treatment providers and social support services
- Health Promotion
- Comprehensive transitional care and re-entry planning
- Attend meeting and trainings as required
- Dispense MAT Medications
