About The Position

Join our Care Coordination team at Moab Regional Hospital! Rural nursing at its finest. You are an individual that enjoys the rapid pace of a thriving family medicine clinic and the excitement and variety of skills that come along with it. This position is a non-exempt, full-time. Anticipated schedule: Monday–Friday. Starting pay range: $36.92-50.88/hour, DOE. A sign-on bonus is available.

Requirements

  • Current RN license in Utah.
  • Excellent time management and organizational skills. Ability to prioritize, multi-task, and problem solve.
  • Knowledge of infection control principles and patient safety techniques. Knowledge of a variety of pharmacological agents used in patient treatments.
  • A positive attitude and the ability to maintain such in situations with changing circumstances.
  • Self-motivation and the ability to be comfortable working both independently and as part of a small team.
  • Comfortable with computers and basic word processing, data entry and electronic medical records.
  • Ability to multi-task effectively in a fast paced work environment.
  • Strong critical thinking skills and ability to problem solve independently.
  • Able to lift/move up to 20 lbs. without assistance and lift/move weights beyond 20 lbs. with assistance.
  • Ability to work in a confidential manner.
  • Reliable and timely with work schedule.
  • Flexible and willing to accept other duties as assigned.

Nice To Haves

  • Bachelor’s degree in Nursing.
  • Two years of experience as an RN.
  • Strong knowledge of community resources in the Moab area.

Responsibilities

  • Promotes preventive care through Annual Wellness Visits
  • Collaborate with health care providers and staff to identify appropriate patients for care management
  • Utilize the nursing process to provide individualized teaching for chronic conditions, lifestyle modifications, and goal setting
  • Provide follow-up in the transitions of care from various settings (hospital or skilled nursing facility discharges and emergency room visits)
  • Maintains accessible, consistent documentation of patient self-management measures, and reporting progress toward goals'
  • Participates in regular team meetings and peer review activities, promotes collaborative teamwork and is able to work with peers in a team situation
  • Has knowledge of community resources available to patients and is comfortable coordinating with outside partners
  • Compiles and abstracts data for multiple clinical measures (PCMH, MIPS, CPC+, etc.)
  • Assist with prior authorizations for medications, procedures, and testing as needed
  • Fill in for staffing needs and other duties as assigned by the Clinic Nurse Manager
