About The Position

St. Mary's Medical Center is seeking a full-time Medical Social Worker (MSW) for its Care Management Department. The Social Worker exercises independent judgment and professional discretion in case management for a selected group of patients. The MSW collaborates with attending physicians and residents and advocates for patients and families as they navigate an acute episode of care. The role includes monitoring patients’ continuing stays for resource utilization and helping prepare them for safe, timely discharge. The MSW uses collaborative problem-solving to meet patient and physician needs and provides a full range of medical social work services as needed, drawing on family, community, state and federal agency, and patient resources to support patients’ return to a productive life.

Requirements

• Master’s degree (MSW) from an approved school of Social Work.
• Current license in the state of West Virginia; temporary permits are not accepted.
• One year of clinical social work experience, community-based and/or hospital-based.
• BLS certification required.

Nice To Haves

• Care Management certification preferred, but not required.
