About The Position

The Medical Social Worker will complete psychosocial assessments for patients' post-hospital care, assisting them and their support systems in coping with the stresses of hospitalization, disability, and chronic/terminal illness. This role collaborates with the Clinical Care Management team to assess and evaluate the appropriate level of care or environment setting to meet patient needs across the continuum. The Medical Social Worker will also serve as a resource to hospital staff and physicians regarding the emotional, social, and psychosocial components of a patient's illness and its effects on their social support system. Additionally, they will provide information and education to patients on community resources and post-hospital care options, make referrals to community agencies, and report suspected cases of child and adult abuse/neglect/exploitation. This role also acts as a liaison between the hospital, patients, and the Department of Social Services during evaluations or investigations and responds to requests for consultative services after normal working hours.

Requirements

  • Master’s degree in Social Work (MSW) required
  • LCSW preferred; applicable state certification preferred
  • Adherence to the National Association of Social Workers (NASW) Code of Ethics
  • At least 1 year of professional experience in a hospital or healthcare-related setting preferred
  • Experience with data management tools.
  • Hospital or acute care management experience preferred
  • Strong assessment, care coordination, and discharge planning skills
  • Ability to work independently and collaboratively within an interdisciplinary team
  • Strong communication skills and adaptability in a fast-paced environment
  • Experience with complex discharge planning
  • Flexibility and reliability for PRN coverage needs

Responsibilities

  • Completes psychosocial assessments for the patient's post hospital care for designated patients and their support system.
  • Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
  • Works with the Clinical Care Management team to assess and evaluate the patient for the appropriate level of care or environment setting to meet care needs across the continuum.
  • Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient's illness and its effects on their social support system.
  • In collaboration with the Clinical Care Management team, provides information, education to patients on community resources and options for post hospital care appropriate to the age of the patients served.
  • Make referrals to community agencies as needed.
  • Reports suspected cases of child & adult abuse/neglect/exploitation.
  • Serves as liaison between hospital, patients and Department of Social Services during evaluation/investigation.
  • Responds to requests for consultative services to patients after normal working hours.
