About The Position

This role is responsible for creating a plan for care across the continuum, integrating patient/family preferences and values. The Social Worker will monitor patient care through assessments, evaluations, and/or patient records. They will advocate for resources and the removal of barriers, and maintain ongoing dialog with supervisors and other health providers to ensure effective implementation of the health plan. Additionally, the Social Worker will act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

Requirements

Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Masters Degree

Nice To Haves

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
