About The Position

Coordinate the overall interdisciplinary plan of care from admission to discharge, integrating patient and family preferences, while requiring an LSW or LCSW to manage the process. Monitor patient care through assessments, evaluations, and medical records while acting as a resource for documentation accuracy and treatment appropriateness. Advocate for necessary resources, remove barriers, and coordinate options for post-discharge care. Maintain ongoing dialogue with supervisors and other healthcare providers to ensure effective implementation of the health plan.

Requirements

  • LSW or LCSW required.
  • Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
