This position is responsible for completing psychosocial assessments for patients' post-hospital care, assisting patients in coping with stress related to hospitalization and illness, and collaborating with the Clinical Care Management team to assess and evaluate the appropriate level of care or environment setting. The Medical Social Worker also serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of a patient's illness, provides information and education on community resources, and makes referrals to community agencies as needed. Additionally, this role involves reporting suspected cases of abuse/neglect/exploitation and serving as a liaison between the hospital, patients, and the Department of Social Services during investigations. The position also requires responding to requests for consultative services after normal working hours.
