The Medical Social Worker will complete psychosocial assessments for patients' post-hospital care, assisting them in coping with stress related to hospitalization, disability, and chronic/terminal illness. This role works within the Clinical Care Management team to assess and evaluate patients for appropriate care levels or settings across the continuum. The Social Worker will serve as a resource to hospital staff and physicians regarding the emotional, social, and psychosocial components of a patient's illness and its effects on their social support system. In collaboration with the team, they will provide information and education to patients on community resources and post-hospital care options, make referrals to community agencies, and report suspected cases of abuse, neglect, or exploitation. They will also act as a liaison between the hospital, patients, and the Department of Social Services and respond to consultative service requests after normal working hours.
