About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The per diem Medical Social Worker provides psychosocial support, assessment, and care coordination to patients and families receiving services at home. The role addresses emotional, social, financial, and environmental factors that affect health and recovery, supports safe and effective care transitions, and works with the interdisciplinary team to connect patients with community resources and provide counseling.
Requirements
- Master's and/or doctorate degree from a school of social work accredited by the Council on Social Work Education.
- Active licensure in Medical Social Work (LMSW) or Clinical Social Work (LCSW).
- One year of social work experience in a health care setting.
- Meet state and federal requirements for the position.
- Current CPR certification.
- Driver's license and access to reliable transportation.
Responsibilities
- Assist patients and families with health-related financial, social, and emotional concerns and help them develop effective coping skills.
- Provide social work services in accordance with the plan of care.
- Communicate changes in a patient's clinical status to healthcare team members and care center office staff in a timely manner.
- Educate patients, family members, and other team members about medical social work interventions and the treatment plan.
- Perform care planning, transfer, and discharge functions.
- Identify risks or threats to patient health in the patient's environment and promptly implement appropriate interventions to reduce them; communicate identified risks to the physician and healthcare team to coordinate care.
- Perform other duties as assigned.
