The Medical Social Worker supports patients and their support systems with psychosocial needs related to hospitalization, disability, chronic illness, or terminal illness. The role assesses post-hospital care needs and works with the Clinical Care Management team to identify appropriate care levels and settings across the continuum. The social worker provides information about community resources and post-hospital care options, makes referrals, and serves as a resource to hospital staff and physicians on emotional, social, and psychosocial aspects of illness. The position also reports suspected abuse, neglect, or exploitation and liaises with patients, the hospital, and the Department of Social Services during evaluations or investigations. It responds to consultative service requests after normal working hours and takes scheduled on-call duty. This is a part-time, benefits-eligible position working 24 hours per week, scheduled Saturday and Sunday from 7:00 a.m. to 7:30 p.m., including holiday weekends. The role works in an office-type setting and throughout the facility, with extensive walking and prolonged periods of sitting while reviewing medical records and completing documentation. It requires frequent verbal and written communication in English, the ability to move independently between locations in a fast-paced environment, and occasional lifting of 10–20 pounds. Repetitive wrist motion is required, as is intact sight and hearing, with or without assistive devices.
