About The Position

The Medical Social Worker provides psychosocial support, assessment, and care coordination to patients and families receiving home-based services. The role addresses emotional, social, financial, and environmental factors affecting health and recovery, supports safe and effective care transitions, and collaborates with the interdisciplinary team to connect patients with community resources, provide counseling, and promote optimal outcomes at home. The patient coverage territory includes Middlesex, Norfolk, and Worcester Counties.

Requirements

• Master's and/or doctorate degree from a school of social work accredited by the Council on Social Work Education.
• Active licensure in Medical Social Work (LMSW) or Clinical Social Work (LCSW).
• Current CPR certification.
• At least 1 year of social work experience in a healthcare setting.
• Meet state and federal requirements for the position.
• Driver's license and access to reliable transportation.

Responsibilities

• Assist patients and families with health-related financial, social, and emotional concerns and help them develop effective coping skills.
• Provide social work services in accordance with the plan of care.
• Communicate changes in a patient's clinical status to healthcare team members and care center office staff in a timely manner.
• Educate patients, family members, and other team members about medical social work interventions and the treatment plan.
• Perform care planning, transfer, and discharge functions.
• Identify risks or threats to patient health in the home environment and promptly implement appropriate interventions to reduce them.
• Communicate identified risks or threats to the physician and other healthcare team members to coordinate care effectively.
• Perform other duties as assigned.
