About The Position

Mercy Haven is opening Long Island's first Medical Respite Program in Bay Shore, a 10-bed facility providing short-term, recuperative care for individuals experiencing homelessness or unstable housing who are recovering from or preparing for surgery. This role offers an opportunity to help people heal with dignity in a safe, supportive environment. The Medical Respite Program Coordinator serves as the primary case manager, responsible for coordinating client care and ensuring individuals receive safe, supportive, and medically appropriate services. The Coordinator collaborates with medical providers, social service agencies, and community partners to facilitate seamless care transitions, improve client outcomes, and support program operations.

Requirements

  • Master's degree in social work or a related field plus 1 year of full time relevant experience OR Associate or Bachelor of Science in Nursing (ADN/BSN) with RN license plus 1 year of full-time relevant experience OR Bachelor's degree in any field with significant coursework or experience in human, social, or health services plus 3 years full-time relevant experience, including 1 year in program operations or management. OR Associate degree plus 4 years of full-time relevant experience, including 1 year in program operations or management. OR High school diploma or equivalent plus 5 years of full-time relevant experience, including 1 year in program operations or management.
  • Valid Current NYS Driver's License in good standing
  • Knowledge of social determinants of health, community resources, and healthcare systems
  • Experience working with Microsoft Word, familiarity with email applications, comfortable using Electronic Health Records required

Responsibilities

  • Conduct comprehensive client intake assessments, eligibility screening, and triage including at site and in the community
  • Develop, implement, and monitor individualized care plans in collaboration with medical, behavioral health, and social service providers
  • Provide ongoing case management to address medical, behavioral health, social, and housing needs
  • Coordinate and monitor access to medical care, including scheduling appointments, facilitating transportation, and supporting adherence to treatment and discharge recommendations
  • Support clients in obtaining identification, benefits (e.g., Medicaid, SSI/SSD), and other essential resources necessary for long-term stability
  • Maintain accurate, timely, and compliant documentation of all client interactions, service plans, and progress notes in accordance with agency policies, regulatory standards, and funder requirements
  • Facilitate discharge planning and ensure warm hand-offs to community resources, including permanent housing, primary care, and supportive services
  • Adhere to Mercy Haven and DOH policies, procedures and regulations
  • Provide on-call coverage as required to support program operations and respond to client needs or emergencies
  • All other relevant duties as assigned
