About The Position

We are seeking a detail-oriented Medical Records Specialist to support a fast-paced Patient Care Plan team within a growing, tech-enabled healthcare organization. This role is ideal for someone who thrives in a high-volume environment, enjoys collaborating with clinical teams, and takes pride in documentation accuracy and timely follow-up. As a Medical Records Specialist, you will play a critical role in ensuring patient records and plans of care are complete, accurate, and compliant—helping drive positive outcomes for patients and providers alike.

Requirements

  • 1+ year of experience with EMR/EHR systems
  • Proficiency in Microsoft Word and Excel
  • 1+ year of customer service experience
  • 1+ year of remote work experience
  • Strong attention to detail and documentation accuracy
  • Ability to manage high volumes of work while meeting performance metrics
  • Task-oriented, self-motivated, and driven by results

Nice To Haves

  • Experience in medical records, healthcare administration, or clinical support
  • Strong communication and follow-up skills
  • Ability to prioritize tasks and meet weekly productivity goals

Responsibilities

  • Manage medical records processes for an assigned geographic area (one or more states)
  • Support 60–80 communities, ensuring all Plans of Care are uploaded within one week
  • Complete 45 in-process Plans of Care per week
  • Communicate with physician offices via phone and fax to secure required documentation
  • Conduct patient record reviews and chart audits to identify missing or incomplete information
  • Coordinate communication between clinical teams and referring providers
  • Organize, transmit, scan, and upload documentation accurately into the EMR
  • Ensure prompt receipt of: Signed initial and recertified Plans of Care Patient Consent to Treat documentation
  • Address billing-related issues tied to missing or incomplete records
  • Respond to phone calls, voicemails, and faxed requests from patients and providers
  • Assist with compliance initiatives, special projects, and other duties as assigned
