About The Position

Under immediate supervision, this role is responsible for maintaining patient files and statistics, responding to requests for medical records, and performing various clerical duties within the Medical Records Department. The position involves ensuring the accuracy and confidentiality of patient information, managing chart analysis, and assisting with the electronic medical record system.

Requirements

  • High school diploma or equivalent.
  • At least one year of experience working as a Medical Records Clerk in a Medical Records Department in an acute care hospital.
  • Must have computer and people skills.
  • Walking, lifting, reaching, hand-eye coordination, speaking.

Responsibilities

  • Maintains established department policies and procedures, objectives, quality improvement and safety programs, environmental and infection control standards.
  • Delivers and retrieves patient charts from in-house clinics and practitioners.
  • Establishes and maintains inpatient and outpatient files and records on an ongoing basis.
  • Performs discharge analysis of inpatient/outpatient and Same Day Surgery charts, noting deficiencies in chart content.
  • Responsible for weekly updating and dissemination of the Deficiency List to physicians and allied health professionals.
  • Answers telephone and gives routine information. Releases information to patients, physicians, attorneys, other hospitals and medical facilities, insurance companies, using appropriate communication methods. Acts as Department Receptionist.
  • Maintains patient confidence by keeping patient records information confidential.
  • Retrieves old medical record numbers for microfilmed MPI cards, when required.
  • Pulls records for off site scanning on a regular basis.
  • Checks permanent files for misfiles to ensure accurate filing.
  • Scans documents into the electronic medical record.
  • Performs chart audits for accurate charges.
  • Ensures charges are added/removed to patient accounts in a timely manner.
  • Assists in care and maintenance of Department equipment and supplies.
  • Participates in educational programs and in-services.
  • Attends meetings as required.
  • Maintains positive and effective relations with co-workers, other departments, patients and visitors.
  • Performs other related duties as assigned or requested.
