About The Position

The Medical Records Clerk manages, organizes, and maintains patient health records, ensuring accuracy, confidentiality, and accessibility for healthcare providers. This role involves retrieving medical records for Providers, Patients, External sources and Staff, and ensuring all records comply with HIPAA and other federal regulations. The clerk is responsible for the closure of medical records, including all required forms and signatures. Clarion Psychiatric Center offers inpatient treatment for adults with moderate to severe psychiatric problems, inpatient crisis stabilization for children and adolescents, and a Partial Hospitalization Program for school-aged children. Our recently expanded 112-bed inpatient program has 4 units. Requirements

  • High School Graduate.
  • Medical Records/HIM experience required.
  • Completion of a Medical Coding course is required.
  • Certification as a Professional Coder (CPC) is required upon completion of Medical Coding course.
  • Three years’ experience in Health Information Management required.
  • Knowledge of medical and psychiatric terminology.
  • Knowledge of DSM-V and ICD-10-CM coding and ability to abstract charts.
  • Must be highly proficient in computer applications and MS Products.
  • Ability to communicate verbally and in writing.
  • Must be able to lift a minimum of 50 lbs. and move frequently in completion of job requirements.
  • Must be able to walk, stand, sit, and read for long periods and reach above and below the waist, turn, twist, and manipulate office equipment.
  • Able to handle a variety of repetitive tasks and multiple assignments under deadlines with short notice.
  • Able to participate in crisis prevention intervention.
  • Drug/Alcohol free as evidenced by negative results of post offer, pre-employment urine drug screen.

Nice To Haves

  • Ability to code diagnoses using DSM-V and ICD-10-CM coding manuals preferred.
  • Valid nursing license required to dictate discharge summaries.

Responsibilities

  • Responding to internal and external customer inquiries.
