About The Position

TISTA is seeking a detail-oriented Medical Record Abstractor to support healthcare quality improvement initiatives. In this role, you will review medical records, abstract clinical data, and ensure accurate data entry into designated abstraction software in accordance with customer specifications and organizational guidelines. Your work will directly support the delivery of high-quality healthcare data used to evaluate care standards, support regulatory reporting requirements and improve patient outcomes. Requirements

  • Minimum of two (2) years of experience in a hospital, healthcare or medical record review environment.
  • Education or formal training in a related field may substitute for experience requirements.
  • Thorough knowledge of medical terminology.
  • Proficiency with Microsoft Office applications.
  • Strong attention to detail with the ability to perform work accurately and efficiently.
  • Ability to prioritize and manage multiple tasks in a fast-paced environment.
  • Ability to work independently and collaboratively within a team environment.
  • Must be able to obtain and maintain a Public Trust clearance.
  • High school diploma or equivalent required.

Nice To Haves

  • Minimum of 1-2 years of hands-on clinical data abstraction.
  • Familiarity with healthcare quality measures and chart review methodologies.
  • Proven ability to meet strict daily chart-volume quotas and accuracy standards.

Responsibilities

  • Review medical records and source documentation to identify and abstract required clinical data elements.
  • Accurately enter abstracted date into designated abstraction software systems.
  • Maintain the confidentiality and security of medical records and protected health information (PHI).
  • Follow established processes, procedures and quality standards.
  • Meet or exceed established productivity and accuracy expectations.
  • Perform additional project-related duties as assigned.
