Medical Mobile Unit Provider - NP or APP
About The Position
Clinica Sierra Vista is seeking a Nurse Practitioner or Advanced Practice Provider to provide acute, preventive, and general primary care services to communities across Kern County. The provider works in a warm, caring environment and is expected to demonstrate experience serving diverse communities, flexibility, knowledge, and compassion for patients. The provider delivers care in a mobile unit at locations such as schools, homeless shelter centers, agricultural sites, and special events throughout Kern and Fresno counties. The role collaborates with other healthcare professionals to provide comprehensive care, maintains accurate patient records, follows medical ethics and quality standards, participates in clinic quality programs, and shares phone-triage responsibilities. The schedule is Monday through Friday, 8 a.m. to 5 p.m.; the provider must also be flexible to work different hours and days depending on organizational needs.

Responsibilities
- Provide acute, preventive, and general primary care services in the mobile unit at schools, homeless shelter centers, agricultural sites, and special events throughout Kern and Fresno counties.
- Work different hours and days as needed to meet organizational needs.

- Participate in staff meetings and provide advice on strategic planning.
- Collaborate with primary care providers, clinical pharmacists, psychiatrists, psychologists, social workers, and other healthcare professionals to provide comprehensive patient care.
- Maintain accurate and up-to-date patient records in compliance with legal, ethical, and organizational standards.
- Adhere to the highest standards of medical ethics.
- Assure quality of care and regularly participate in clinic Peer Review and Quality Assurance Programs.
- Share phone-triage responsibilities with other Clinica Sierra Vista providers.
- For MDs and DOs only: supervise advanced practice providers, including nurse practitioners and physician assistants, working in clinics on-site or by telephone.
