About The Position

Liberty Resources, Inc. is seeking a Board-Certified Physician to serve as the Medical Director of Primary Care for our Federally Qualified Health Center (FQHC) located in Syracuse, NY. This physician leader will provide both direct patient care and clinical oversight while helping advance an innovative integrated care model that combines medical and behavioral health services. Working closely with clinical and operational leadership, the Medical Director will support quality improvement, provider development, patient safety initiatives, and practice growth while maintaining a strong commitment to evidence-based and patient-centered care. As one of only a limited number of organizations nationally designated as both a Certified Community Behavioral Health Clinic (CCBHC) and FQHC, Liberty Resources offers a unique opportunity to transform healthcare delivery for underserved communities within a highly collaborative and sustainable system of care. The Medical Director will initially maintain a significant clinical practice while supporting administrative and leadership functions. As the practice continues to grow, the balance of clinical and administrative responsibilities may evolve.

Requirements

MD or DO from an accredited medical school.
Board Certified in Family Medicine, Internal Medicine, or Med/Peds.
Active and unrestricted New York State medical license.
DEA registration or ability to obtain prior to employment.
Minimum of five years of clinical primary care experience.

Nice To Haves

Previous physician leadership experience preferred, including provider supervision, quality oversight, or medical directorship responsibilities.
Experience working in integrated care environments preferred.
Strong understanding of quality improvement, population health, and value-based care principles.
Experience serving Medicaid, Medicare, uninsured, and underserved populations preferred.
Experience in an FQHC, community health center, or similar setting preferred.
Demonstrated ability to lead change, drive innovation, and foster continuous improvement.

Responsibilities

Provide direct patient care in an outpatient primary care setting.
Deliver comprehensive medical services within your specialty, including Family Medicine, Internal Medicine, or Med/Peds.
Diagnose, treat, and manage acute and chronic medical conditions.
Perform preventive care, annual wellness visits, and health maintenance services.
Collaborate with behavioral health clinicians to provide integrated care for patients with co-occurring medical and behavioral health conditions.
Participate in case conferences and interdisciplinary team meetings.
Coordinate care with specialists, hospitals, and community providers.
Maintain accurate and timely documentation within the electronic medical record.
Participate in backup telephonic on-call responsibilities.
Demonstrate clinical excellence and serve as a role model for evidence-based, patient-centered care.
Provide clinical leadership and oversight for primary care services.
Foster a culture of excellence, accountability, teamwork, and patient-centered care.
Support provider recruitment, onboarding, mentoring, and retention efforts.
Supervise and support Advanced Practice Providers (APPs) in clinical decision-making and professional development.
Lead quality improvement initiatives and monitor clinical performance metrics.
Ensure compliance with FQHC, HRSA, CCBHC, state, federal, and accreditation requirements.
Participate in quality assurance, incident management, and patient safety activities.
Conduct medical chart reviews and support clinical documentation improvement efforts.
Develop and implement clinical policies, protocols, and evidence-based best practices.
Partner with organizational leadership to advance strategic initiatives and practice growth.
Participate in operational planning, budgeting, and resource allocation discussions.
Provide clinical input regarding healthcare technology, electronic medical records, and workflow improvements.
Lead provider meetings and facilitate effective communication among clinical teams.
Support provider performance management, coaching, and professional development.
Assist in the development of clinical education programs for APPs, medical students, residents, and other learners.
Establish and monitor quality, productivity, and patient satisfaction goals.
Serve as a physician representative within the community and with healthcare partners, hospitals, and professional organizations.
Promote population health initiatives focused on prevention, chronic disease management, and health equity.
Collaborate with leadership to identify and develop new programs, services, and care delivery models.
