About The Position

Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients across multiple states, including New York, New Jersey, Ohio, Pennsylvania, Maryland, DC, Virginia, Alabama, Georgia, Florida, Kansas, Missouri, Texas, Mississippi, and Oklahoma. Ennoble Care's clinicians visit patients' homes, assisted living facilities, or skilled nursing facilities, providing a continuum of care for those with chronic conditions and limited mobility. Ennoble Care offers a variety of programs, including remote patient monitoring, behavioral health management, and chronic care management, to ensure our patients receive the highest-quality care from a team they know and trust. Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
  • Board certification (or eligibility) in Internal Medicine or Family Medicine.
  • Active, unrestricted Ohio medical license (or ability to obtain).
  • Strong clinical skills in diagnosing and managing complex, age-related conditions.
  • Experience in long-term care, skilled nursing facilities, home health, or palliative care is strongly preferred.
  • Familiarity with interdisciplinary care models and comfort working closely with nurse practitioners, physician assistants, and other members of the care team.
  • Knowledge of geriatric syndromes, mobility and fall-risk assessment, and cognitive evaluation tools.
  • Not excluded from participation in Medicare or other Federal Health Care Programs.
  • Excellent verbal and written communication skills, with strong empathy and teamwork abilities.
  • Valid driver's license and ability to travel locally to patients and facilities in the Youngstown, Ohio area.

  • Fellowship training in Geriatric Medicine is preferred.

Responsibilities

  • Provide direct medical care to a panel of older adults across clinic, long-term care, assisted living, and home settings, delivering high-quality, patient-centered geriatric medicine.
  • Conduct comprehensive geriatric assessments, including medical, functional, cognitive, and psychosocial evaluations, to guide individualized care planning.
  • Diagnose and manage complex, chronic conditions common in older adults – including dementia, frailty, cardiovascular disease, osteoporosis, and diabetes – and monitor and adjust medications with attention to polypharmacy and age-related pharmacologic considerations.
  • Develop individualized treatment plans emphasizing prevention, health maintenance, and patient and caregiver education.
  • Guide patients and families through end-of-life care planning, advance directives, and palliative care coordination when appropriate.
  • Coordinate care with Ennoble Care's multidisciplinary team, including nurses, social workers, therapists, pharmacists, and other specialists, and communicate effectively with patients, families, and caregivers on goals of care and quality-of-life concerns.
  • Collaborate with and support nurse practitioners and physician assistants delivering care to the shared patient panel.
  • Support the Regional Chief Medical Officer in the development and reinforcement of clinical protocols and best practices for geriatric care, and contribute geriatric-specific clinical expertise to case reviews and provider training as needed.
  • Participate in quality improvement initiatives and clinical research as directed by the RCMO, and maintain timely, accurate clinical documentation in compliance with regulatory, ethical, and organizational standards.
  • Maintain continuing medical education relevant to geriatric and home-based primary care.
