About The Position

We are a mission-driven PACE organization dedicated to caring for Colorado's most vulnerable seniors — helping them live independently in their communities with dignity and quality of life. This is medicine as it should be: meaningful, unhurried, and deeply human. This role involves community-based physician rounding in subacute skilled nursing facilities, with flexible scheduling allowing you to control your day. You will see 10-20 patients at a time, with only 2 call days per month and a $5,000 annual CME allowance. The Medical Director will lead clinical oversight and quality for PACE provider services, collaborate with an experienced, supportive interdisciplinary team, and serve as a physician leader and mentor to NPs and other providers, championing person-centered care for frail elderly participants.

Requirements

• MD or DO; licensed to practice in Colorado
• ABMS Board Certified — Internal Medicine or Family Medicine (geriatrics preferred)
• DEA registration; ability to maintain hospital privileges
• Experience in primary care with frail or elderly populations
• Passion for mission-driven, whole-person care
