Medical Director, Musculoskeletal (MSK) Programs

Devoted Health•Waltham, MA
•$280,000 - $305,000•Remote

About The Position

Devoted Health is redefining healthcare and seeking a physician leader to build its national Musculoskeletal (MSK) practice. This new clinical program will leverage Devoted Medical's AI platform and be delivered to an engaged member population. MSK issues are a significant source of pain, disability, and avoidable spend for seniors, and the traditional system often leads to unnecessary imaging, injections, and surgery. Existing virtual MSK solutions often fail to serve frail, multimorbid, or post-operative individuals. Devoted Health aims to create a better model, led by a physician who will define the care model, translate best practices into a scalable AI system, and build a team that provides a high-trust, high-empathy human overlay. This is a player-coach role where the individual will practice medicine within the designed model, impacting tens of thousands of members. The role benefits from Devoted Medical's existing virtual and in-home clinical programs, AI platform, and engaged member base. The work involves integrating existing successful elements with new MSK-specific components.

Requirements

  • Board-certified physician in Physical Medicine and Rehabilitation, Sports Medicine, Rheumatology, Orthopedic Surgery, or Internal Medicine / Family Medicine with a substantial MSK practice.
  • Significant clinical experience managing common MSK conditions in adults, including older adults with multimorbidity.
  • Demonstrated experience building or substantially redesigning a clinical program, pathway, or service line (not just running one).
  • A deep belief in measuring and improving quality across a population, and comfort being held accountable to outcomes.
  • Experience serving diverse patient populations, with care that aligns with the mission to treat every member like family.

Nice To Haves

  • 5+ years of experience delivering MSK care in a sports medicine, PM&R, or orthopedics practice.
  • Experience supervising Advanced Practice Providers, physical therapists, or other clinicians.
  • Direct experience working with product and engineering teams to build clinical software or AI-enabled workflows.
  • Experience with virtual or hybrid care delivery.
  • Familiarity with Medicare Advantage populations and value-based care economics.
  • Experience with peri-operative optimization, post-acute recovery, or falls and fracture prevention programs.

Responsibilities

  • Design the care model, including defining which conditions to address, member entry and triage processes, first visit protocols, escalation to imaging or surgical consultation, pain management strategies (avoiding opioids), and peri-operative optimization and post-operative recovery.
  • Define precise care pathways for high-burden conditions such as low back and neck pain, hip and knee osteoarthritis, shoulder pain, fragility fracture and osteoporosis, and falls and deconditioning.
  • Collaborate with product and engineering teams to build an agentic interface for care delivery, encoding clinical reasoning, specifying system interactions, and supervising its performance.
  • Shape the member experience with the AI system, focusing on engagement between visits, adherence to home programs, and early detection of worsening conditions.
  • Practice medicine within the designed model, carrying a shared patient panel and serving as the clinical backstop for diagnostic ambiguity, red flags, complex multimorbid presentations, and non-pathway cases.
  • Hire, train, and lead clinicians including Advanced Practice Providers, physical therapists, and care team members.
  • Set the standard for Devoted MSK encounters, manage collaborative practice agreements across states, conduct case reviews and interdisciplinary rounds, and scale the team nationally while maintaining trust and empathy.
  • Build quality measures, a balanced scorecard, and feedback loops for the program, focusing on function and pain outcomes, appropriate imaging and surgical referral, opioid stewardship, and member experience.
  • Use quality data to iterate on the model and the AI system rapidly.
