About The Position

This role involves ensuring compliance with federal and state regulations, maintaining accurate medical documentation, performing patient care duties, and coordinating health services. The Medical Coordinator will also communicate with leadership, participate in protocol development, and maintain confidential health records. A key aspect of the role is recognizing and responding to signs of abuse, neglect, and exploitation, and adhering to safety protocols. The position requires strong organizational skills, ethical conduct, and effective communication.

Requirements

  • Fluency in English and Spanish is required.
  • Graduate of an accredited school of nursing.
  • Current, unrestricted LVN or RN license in the state program is located.
  • Current BLS certifications (PALS preferred).
  • First aid certification.
  • CPR certification.
  • Minimum of one (1) year employment experience in public health or a healthcare setting with demonstrated experience in care coordination.
  • Pass a pre-employment drug screen and random drug screens throughout employment.
  • Provide proof of work eligibility status upon request.
  • Pass a pre-employment and biennial criminal background checks.
  • Respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
  • Work collaboratively with other staff members, service providers and professionals.
  • Provide crisis intervention according to training provided by Compass Connections when needed to maintain a safe environment.
  • Work in a fast-paced environment and maintain emotional control and professional composure at all times.
  • Maintain computer literacy required to meet the responsibilities of the position.
  • Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
  • Demonstrate a working knowledge of all Compass Connections policies and procedures.
  • Must provide current physical-medical release and provide proof of immunizations, titers, or exemptions for the following: MMR, Varicella, HepB, HepA, Tetanus, PPD.
  • Must be able to perform physical activity such as extensive walking, lifting of patients, stretching, and bending.
  • Must be able to work in a team environment.
  • Must be able to maintain professional people skills, oral, written, listening, and non-verbal communication skills.

Nice To Haves

  • Bachelor’s degree in relevant field.
  • Pediatric nursing experience.
  • PALS certification.
  • Meningococcal immunization (preferred).

Responsibilities

  • Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
  • Have extensive knowledge of medical documentation to be able to generate, decipher, and correct for proper documentation methods in accordance with federal guidelines.
  • Validate the completion of all clinic duties relevant to this position in compliance with all federal guidelines and program protocols via daily audits of Portal documentation consistent with the medical clearance process.
  • Perform duties and tasks to ensure patient safety by assessing and monitoring health conditions, assist with procedures and operate medical equipment, administer vaccines, medications and treatments ordered by medical provider, monitor for side effects and reactions, and provide teaching patients and staff.
  • Validate systems of records for required facility documentation and reporting to meet Compass Connections, federal and state guidelines within the required timeframes.
  • Effectively communicate, consult, and interact with the Lead Medical Coordinator and Regional Director on policy and procedure issues affecting the quality of health care services on campus.
  • Participate in the development of health and medical related protocols and procedures for federal program.
  • Participate in scheduled meetings, team decisions and operations. This includes ensuring appropriate communication of health needs to case managers and clinicians, scheduling health-care appointments, coordinating transportation to health-care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health-care providers.
  • Maintain complete and confidential health care records and information in keeping with information privacy and Protected Health Information (PHI) requirements and Compass Connections confidentiality protocol and procedures.
  • Know and recognize the signs of abuse, neglect, and exploitation and sexually provocative behavior and implement professional response, reporting and documenting procedures.
  • Plan, organize, and prioritize daily work using sound judgment.
  • Knowledge of Compass Connections safety protocols including evacuating with children and other staff in case of an emergency.
  • Maintain confidentiality in all areas of the service population and program operations.
  • Maintain Compass Connections professional and ethical standards of conduct outlined in Compass Connections employee handbook including demonstrating respect for agency staff, children, and community members and complying with required dress code at all times.
  • Collect, enter, and ensure documentation for medical staff at designated site(s), to ensure all required duties meet or exceed program guidelines and best practices established by State and Federal partners.
  • Supervise and coordinate, on occasion, medical staff to perform clinic duties as directed by Lead Medical Coordinator, as well as perform clinic duties as needed.
  • At designated site(s), direct, and train medical staff, and coordinate services, following federal, state, and Compass Connections established policies, procedures, and guidelines.
  • Monitor established health care plans for service population at the site(s) within the timeframe established by our State and Federal partners.
  • Ensure accurate documentation for designated site(s) in accordance with agency policies and procedures, contract and regulatory requirements and standards and program timeline requirements.
  • Meet all program and federal partners’ documentation and processes requirements.
  • Other duties as assigned.
