About The Position

ForzaCare is seeking a Medical Case Manager to help injured workers navigate recovery and return to work. The role serves as a central point of coordination among injured workers, medical providers, employers, and insurance carriers, ensuring care management is timely, transparent, and effective. The position is suited to licensed nurses or certified rehabilitation counselors. Prior workers’ compensation experience is strongly preferred.

Requirements

• Active Registered Nurse (RN) or Certified Rehabilitation Counselor (CRC) license, with an associated college degree.
• Comply with all state-specific licensure and certification requirements.
• Valid driver's license, reliable transportation, and auto insurance, with the ability to travel to appointments.
• Motivated, organized, collaborative, and committed to high-quality, compassionate care and safety, ethical, and professional standards.

Nice To Haves

• Prior experience in workers' compensation case management.
• Additional certifications such as CCM, CIRS, or other case management credentials.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, which may include visiting employers and workers at their place of employment.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Communicate consistently and provide detailed progress reports to clients and stakeholders.
• Ensure case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
