Medical Case Manager - Workers' Compensation
About The Position
ForzaCare is a purpose-driven organization founded in 2022 that helps injured individuals recover faster and return to work safely through coordinated, clinically appropriate care. It is part of Ethos Risk Services, a national provider of investigative and risk mitigation solutions. As a Field Medical Case Manager, you will help injured workers navigate recovery and return to work. You will act as the central point of coordination among injured workers, medical providers, employers, and insurance carriers to support timely, transparent, and effective care management. The role is suited to licensed nurses or certified rehabilitation counselors. Workers' compensation experience is strongly preferred.
Requirements
- Active Registered Nurse (RN) or Certified Rehabilitation Counselor (CRC) license with an associated college degree.
- Comply with all state-specific licensure and certification requirements.
- Valid driver's license, reliable transportation, and auto insurance, with the ability to travel to appointments.
- Demonstrate motivation, organization, collaboration, clear communication, commitment to high-quality compassionate care, and compliance with safety, ethical, and professional standards.
Nice To Haves
- Prior experience in workers' compensation case management.
- Additional certifications such as CCM, CIRS, or other case management credentials.
Responsibilities
- Coordinate care among medical providers, employers, insurance carriers, and injured workers.
- Attend appointments with injured workers, which may include visiting employers and injured workers at their place of employment.
- Develop, document, and monitor individualized recovery goals and return-to-work plans.
- Provide consistent communication and detailed progress reports to clients and stakeholders.
- Ensure case management work meets or exceeds customer and compliance requirements.
- Build and maintain strong relationships with clients, providers, and internal team members.
