About The Position
Provides comprehensive HIV case management and prevention services to individuals living with HIV and those at high risk for HIV infection. Manages a caseload of up to 65 HIV-positive clients, including emergent case management for EIP patients, conducting assessments, and developing and monitoring individualized care plans in accordance with AACO and NJDHSS standards. Ensures clients are linked to appropriate medical, behavioral, and support services while addressing immediate and ongoing needs through referrals and care coordination.
Requirements
- 1 year or more experience in the field of Case Management or Social Services.
- Knowledge of available financial and social service resources in southern New Jersey.
- Excellent interpersonal skills
- Bachelors Degree Social Work, Counseling, Psychology, or a related field
Nice To Haves
- Experience in the field of HIV preferred.
- AACO or NJ HIV Case Management Certification preferred.
