About The Position

The Care Coach Connect Medical Assistant will provide a variety of patient care activities supporting the needs of the Care Coach Connect Program. This role involves clinical duties in patients' homes, as well as some office/clerical duties. The position organizes the clinical environment and provides support in patient care situations, including assisting physicians and nursing personnel with various procedures under the supervision of an Advanced Practice Clinician or medical provider. The Medical Assistant will assist in identifying patient needs or problems and communicating data to the provider or other members of the clinical team, while delivering quality customer service and ensuring adherence to infection control, privacy, and confidentiality policies.

Requirements

  • High school graduate or GED equivalent
  • Current, nationally recognized Medical Assistant certification or registration or must obtain the designation within 90 days of employment hire date
  • Current CPR certification or must obtain certification within 30 days of employment hire date
  • Demonstrated basic computer literacy
  • Proven knowledge of medical terminology
  • ICD-10 and CPT coding experience
  • Proven ability to react calmly and effectively in emergency situations required and assume a leadership role until higher level clinician can arrive on the scene
  • Proven ability to travel frequently within assigned market
  • Proven ability to be flexible and adaptable to change
  • Access to reliable transportation for daily travel to various clinics as assigned
  • Valid driver’s license and current automobile insurance
  • Ability to travel, approximately 80% of the time, to meet with members in their homes, the hospital or long-term care setting, including overnight travel
  • This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease

  • 3+ years of experience as a Medical Assistant or a combination of 5+ years of non-certified medical assistant experience and certified medical assistant experience

Responsibilities

  • Performs all duties within the scope of a Certified Medical Assistant (provider supervised procedures, injections, diagnostic testing, phlebotomy)
  • Coordinates healthcare screening results as directed by providers, company standards, and policies
  • Travels with provider to patient homes, mobile units, and other locations within the NTX Market and surrounding NTX Market to support the delivery of services in the community setting
  • Performs and completes all quality program health screenings per required metrics
  • Processes appropriate documents in an organized and accurate manner and copies all physicians on patient results and quality forms submitted
  • Records patient care documentation in the medical records accurately and in a timely manner according to company policy
  • Reviews all medical information, test results and survey questions prior to the patient leaving to ensure all information is captured
  • Ensuring that supplies are available, stock is inventoried, log data is accurate and supplies are rotated. Follows up with discrepancies
  • Presents abnormal results to the provider promptly to ensure there is not an emergent condition that needs immediate attention
  • Post visit preparation of the clinical team charts for faxing, filing and forwarding to the PCP offices
  • Responsible for ensuring the completion of compiling results and entering chart notes prior to visit. This includes, but is not limited to, pulling hospital records, lab results, specialty notes, etc.
  • Ensures that follow-up appointment dispositions are documented in CDSS and completed in eCW
  • Reviewing and ensuring that post-visit recommendations from provider are completed such as prescriptions and referrals
  • Maintains patient confidentiality of all materials handled within the department as well as proper release of patient information at all times
  • Maintains certifications (MA and CPR) and quality control standards
  • Promotes positive work environment
  • May assist other members of their team with coordination of work
  • Performs all other related duties as assigned
  • Operates diagnostic equipment (cannot interpret tests), assists in removal of staples from superficial wounds, changes wound dressing and obtains cultures, administers non-intravenous medication, performs simple specimen collection via non-invasive techniques and collects blood specimens via venipuncture or via capillary, performs EKGs. Performs quality control checks on equipment
  • Ensuring Specialist/Home Health/DME referrals are entered and followed up in a timely manner
  • Assisting in development of Plan of Care paperwork for the assigned Advanced Practice Clinician to review
