About The Position

Hamilton Park Nursing and Rehab is looking for a Medicaid Coordinator to join the team at our Skilled Nursing Facility in Brooklyn, NY! The Allure Group is a consortium of six rehabilitation centers across Brooklyn and Manhattan, NY, specializing in post-acute and sub-acute skilled nursing and rehabilitation. Requirements

High school diploma or its equivalent
Bachelor’s Degree in Business, Accounting/Finance, Marketing, Social Work , or a related field
Previous experience in a long-term care facility or another healthcare-related facility

Responsibilities

Screen, interview, and assess each resident to determine Medicaid benefit eligibility
Receive and review admission forms for residents referred for Medicaid assistance
Collect all necessary documents to apply for Medicaid
Upload documents to be sent electronically to the Medicaid office
Correspond with HRA case workers to get the status of Medicaid applications
Follow up to ensure that all applications are approved
Obtain necessary information to determine eligibility for financial aid
Secure and complete appropriate forms in accordance with the established procedure
Perform other job-related duties as directed by Supervisor
