About The Position

Z-Radar is looking for a full-time MDS Support Specialist in the state of New York to join our organization. We are looking for someone who can work independently and confidently and is a great team player.

Requirements

Must be proficient with PDPM and case mix reimbursement.
Must have experience working with Quality Measures and Five Star.
Must have good computer skills, be knowledgeable with PCC and Net Health, and be willing to learn new EMR’s.
NY License Required

RAC-CT preferred.

Responsibilities

Complete MDS assessments including, Medicare PPS and COBRA required assessments, as directed by the VPs of Clinical Services or other R2 Leadership.
Responsible for accurate and timely completion of assessments, in full compliance with regulations and guidelines set forth by CMS and as instructed by the Resident Assessment Instrument (“RAI”) Manual.
Complete comprehensive Care Area Assessments (“CAAs”) or other Care planning requirements, as directed by Leadership.
Attend interdisciplinary meetings and effectively communicate the assessment status to Facility team.
General activities, as agreed, are consistent with industry norms specific to this position.
