One of LSC’s senior living communities, Trinity Oaks Health & Rehab, is seeking a full-time MDS Nurse. As an MDS Nurse at Trinity Oaks, you will work collaboratively to develop, organize, and monitor individualized care plans for residents from admission through discharge. You’ll coordinate with an interdisciplinary team, residents, and families to ensure our care meets both state and federal standards.
Requirements
- Graduation from an accredited RN program.
- Current RN license in the state of North Carolina.
- CPR certification or willingness to complete it.
Responsibilities
- Develop, organize, and monitor individualized care plans for residents from admission through discharge.
- Coordinate with an interdisciplinary team, residents, and families to ensure care meets both state and federal standards.
