About The Position
Diablo Post Acute is seeking an experienced and compassionate Registered Nurse (RN) – Minimum Data Set (MDS) Coordinator to join our dedicated team! This is a full-time position.
Requirements
- Current RN license in California.
- Strong knowledge of Medicare, Medicaid, and CMS regulations.
- Excellent organizational, communication, and documentation skills.
- Ability to work collaboratively in a fast-paced environment.
Nice To Haves
- Previous MDS experience in a skilled nursing facility preferred.
Responsibilities
- Coordinate and complete MDS assessments in accordance with state and federal regulations.
- Ensure accurate and timely completion of resident assessments and care plans.
- Collaborate with the interdisciplinary team to optimize resident outcomes.
- Maintain compliance with CMS guidelines and documentation requirements.
- Participate in quality improvement initiatives and survey readiness.
