Requirements

• Active RN or LPN in the state of Washington
• Long term care experience
• Experience with MDS process and Case Mix Management
• Knowledge of regulatory guidelines

Responsibilities

• Ensuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.
• Identifying resident acuity, helping to determine specific care needs and communicating those needs to the care team as well as the residents’ families and responsible parties.
• Coordinating with other disciplines and training staff on the RAI process as needed.
• Transmitting completed assessments.
• Participating in the weekly Medicare meeting, daily clinical meeting and the monthly QAPI meeting.
• Communicating with insurance companies and providing requested information to obtain continued authorization for care as the Case Manager.
