About The Position

Gateway Post Acute is seeking a knowledgeable and organized MDS Nurse – Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to join our interdisciplinary team. This full-time position is ideal for a detail-oriented nurse who is committed to accurate resident assessments, regulatory compliance, and high-quality care.

Requirements

Current and active California LVN or RN license.
Strong organizational, communication, and time-management skills.
Ability to work effectively with an interdisciplinary healthcare team.
Excellent attention to detail and commitment to accurate documentation.

Nice To Haves

Previous experience in skilled nursing, long-term care, or post-acute care preferred.
MDS and care planning experience preferred.
Knowledge of Medicare, Medicaid, PDPM, and regulatory requirements is a plus.

Responsibilities

Coordinate and complete accurate MDS assessments within required timeframes.
Collect, review, and verify resident clinical information.
Ensure MDS documentation complies with federal, state, and facility requirements.
Monitor assessment schedules and maintain timely completion of all required documentation.
Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.
Participate in care plan meetings and assist with the development of individualized resident care plans.
Review medical records to ensure documentation supports resident needs, services, and reimbursement.
Identify documentation concerns and provide education to clinical staff as needed.
Maintain the confidentiality of all resident and facility information.
Support quality assurance and performance improvement initiatives.
