About The Position

Checotah Nursing and Rehabilitation is seeking an MDS Coordinator! Candidate must hold an active LPN or RN license in the state of Oklahoma, with no disciplinary action. Previous experience in an administrative nursing position is preferred. Requirements

• LPN or RN license in the state of Oklahoma
• Attention to detail to ensure precision in completing assessments and accurate data entry
• Assessment, data collection and documentation skills
• Written and verbal communication skills
• Organizational skills
• Comfort with electronic records
• Understanding of Medicare and Medicaid requirements
• An understanding of medical terminology, nursing practices and patient care standards

• Previous experience in an administrative nursing position

Responsibilities

• Completing and updating MDS forms on all patients in the care facility
• Performing clinical assessments to gather information on residents
• Developing care plans for patients to coordinate available services
• Submitting updates and paperwork to maintain compliance
• Participating in the admission and discharge processes
• Conducting and attending meetings with other care team members
• Investigating complaints or issues from residents and their families
• Educating patients and their families
